CLMS Conference Registration

This is not an electronic form. To register, complete the application below, print and fax or mail it. DO NOT BOTH FAX AND MAIL.

Be sure to review the cancellation policy. PLEASE USE ONE FORM PER PERSON.

Purchase order registrations must include an actual copy of the purchase order.

Registration Instructions
(Only purchase orders or credit card orders may be faxed.)

Regular Registrations - must be received with payment by mail or FAX in order to be processed.

On-Site Registration - after the "No Refund" date (see cancellation policy), you must register on-site (there is no additional cost). Payment by cash, check, purchase order, Visa or Mastercard will be accepted.

To ensure you have met strict payment deadlines, please:

  • Fax your credit card order or purchase order at least three days before the "No Refund" date (see cancellation policy), then call CLMS at (562) 430-3136 to confirm your fax has been received.
  • If you must mail a check, please allow 10 days for mail delivery before the "No Refund" date (see cancellation policy). CLMS is not responsible for delayed mail.
  • DO NOT both fax and mail payment information.

Cancellation Policies

Cancellations must be in writing and faxed to (562) 430-5607. Please call as soon as you fax your request to verify receipt at (562) 430-3136. A colleague may substitute for you anytime to avoid cancellation penalties.

Prior to 1/29/09 - Full refund
1/29/09 to 2/18/09 - $60 fee
2/19/09 to 2/26/09 - 50% fee
A fter 2/26/09 - No refund, but a colleague may substitute
